• Breast Revision Surgery

  Think of this as a “touch-up”, either major or minor. A breast revision can include an implant exchange, replacement, or removal. It may involve lifting or re-lifting the breast tissue or moving an implant to a better position, or improving the appearance of the scar. Any aspect of your prior surgery that is no longer satisfactory can also be addressed by breast revision surgery. Am I A Good Candidate For Breast Revision Surgery? • Breast Lift (Mastopexy)

  What Is A Breast Lift? Also known as mastopexy, a breast lift is a surgical procedure performed to raise and reshape breasts that have sagged as a result of aging, pregnancy, weight loss, heredity, gravity or other factors. By trimming excess skin and tightening supporting tissue, patients can achieve an uplifted, youthful breast contour. After surgery, the breasts are higher on the chest and firmer to the touch. • Breast Asymmetry

  Many women are genetically predisposed to having breasts that are uneven in size and shape. These differences in breasts are often undetectable to patients themselves, causing no problems at all, while other women can have obvious asymmetry, which can cause both an unbalanced appearance and emotional distress. Asymmetric breasts can be corrected through breast surgery that either enlarges the smaller breast or reduces the larger one. Implants can be placed in smaller breasts or a breast reduction can be performed on larger breasts. If the nipples appear at different heights, surgery can relocate one or both of the nipples. All of these options produce breasts that are more similar in appearance, working with each woman’s unique body to achieve the best possible results. • Breast Reduction

  Large breasts can negatively impact a woman both physically and emotionally, causing symptoms such as chronic back pain, rashes, poor posture, low self-esteem and restricted activity. Many women seek treatment to reduce the painful effects of their large breasts, while also improving their self-confidence and overall quality of life. Also known as reduction mammaplasty, breast reduction surgery reduces the size of large, disproportionate breasts by removing excess fat, tissue and skin for a more desirable appearance and relief of physical and emotional symptoms. During the breast reduction procedure, the surgeon makes either a circular incision around the nipple then downwards or an anchor incision that circles the nipple and extends down the breast and along the breast crease. Fat and glandular tissue is then removed, while excess skin is tightened to produce a smaller, natural-looking breast. The nipple and areola are not removed but are usually repositioned higher on the breast. The results of a breast reduction are visible right away and will continue to improve as swelling and bruising subside. Scars will continue to fade over time, becoming less and less noticeable. One of the most commonly asked questions patients have is whether insurance will cover any portion of breast reduction. The answer is, it depends. It depends on your insurance provider. It depends on whether the size of your breasts is causing other health concerns (e.g. back or neck problems), and it depends on how much tissue may need to be removed. Insurance companies typically require documentation that you’ve tried to correct any issue with physical therapy prior to seeking plastic surgery.

• Breast Reconstruction

  Modern surgical technology makes it possible to construct a natural-looking breast after mastectomy (breast removal) for cancer or other diseases. The procedure is commonly begun and sometimes completed immediately following mastectomy so that the patient wakes with a new breast mound instead of no breast at all. Alternatively, reconstruction may begin years after mastectomy. Many insurance companies cover reconstruction following breast cancer surgery, and legislation is currently before Congress to make coverage mandatory. Women whose cancer seems to have been eradicated with mastectomy are the best candidates for breast reconstruction. Those with health problems such as obesity and high blood pressure and those who smoke are advised to wait. Others prefer to postpone surgery as they come to terms with having cancer, consider the extent of the procedure, or explore alternatives. The reconstruction itself consists of multiple operations, the first of which involves the creation of a breast mound and is performed during or after mastectomy in a hospital under general anesthesia. Later surgeries, if necessary, may be done in the hospital or an outpatient facility, with either general or local anesthesia. There are several ways to reconstruct the breast, both with and without implants; your breast surgeon and plastic surgeon should work together with you in deciding which approach is best for you. Breast reconstruction has not been proven to affect the recurrence of cancer or other diseases, chemotherapy or radiation treatment. Nevertheless, in addition to the complications possible from any surgical procedure (bleeding, fluid collection, excessive scar tissue, or difficulties with anesthesia), there are some risks inherent in breast reconstruction, including infection around the implant, if an implant is used, and capsular contracture, when the scar (capsule) around the implant tightens, causing the breast to feel hard. Treatment for capsular contracture varies from “scoring” the scar tissue to removing or replacing the implant. Some patients may need time to come to terms emotionally with their new breasts.

• Male Breast Reduction (Gynecomastia)

  Gynecomastia is a common condition that affects nearly half of all men, resulting in localized fat and/or glandular tissue in the breasts. For most cases of gynecomastia, the cause is unknown, but this condition may be linked to certain medications or diseases. Many men are embarrassed by this condition and seek treatment to achieve a smooth, contoured chest through male breast reduction. Male breast reduction may be performed using liposuction, surgery or a combination of the two, depending on the amount and type of tissue found in the breasts. If the breasts consist of mostly fatty tissue, liposuction may be used to suction out fat from an incision in the nipple or underarm areas. For breasts with an excessive amount of glandular tissue, excision surgery may be performed, which requires cutting away the excess fat, skin and tissue through a larger incision. Your surgeon will determine which technique is best for you based on your individual goals for surgery. After breast reduction surgery, patients will usually be able to return home the very same day. Post-operative symptoms may include swelling and discomfort, which can be managed through pain medication and compression garments worn for the first few days after surgery. Patients will be able to return to work once they feel well enough, but should avoid sexual activity and strenuous exercise for a week or two while the chest heals. For most patients, breast reduction surgery produces a flatter, more well-defined chest that can last for many years, as long as patients maintain a stable weight.   What Is A “Mommy Makeover”? Also known as a post-pregnancy makeover, the “Mommy Makeover” refers to a series of procedures designed to restore a woman’s figure after bearing children. This may include cosmetic surgery of the breast, such as augmentation, lift, or reduction, in conjunction with cosmetic surgery of the abdomen, such as one of the tummy tuck procedures. • Abdominal Liposuction

  The abdomen is often the site of localized fat accumulation that may result in a bulge or prominence. Many patients are frustrated by their inability to correct this completely with diet and exercise. Liposuction will remove this fat from underneath the skin without leaving significant scars, resulting in an improved abdominal contour. Recovery times are minimized by this approach as well. However, the skin must have sufficient elasticity to tighten after the fat is removed. Patients with many stretch marks or generally loose or flabby skin may be better candidates for abdominoplasty.

• Arm Lift (Brachioplasty)

  An arm lift, also known as brachioplasty, is a surgical procedure performed to remove excess fat and skin from the upper arms for a more toned, slimmer appearance. Over time, the soft tissue in the upper arm becomes lax and may begin to droop and sag, resulting in arms that appear loose and hanging. • Arm Liposuction

  Liposuction is a minimally invasive cosmetic procedure that can be used to remove stubborn fat and tissue from stubborn areas of the body, including the upper arms, which have a tendency to sag with age or significant weight gain or loss. The liposuction procedure can achieve a slimmer contour with minimal scarring by inserting a cannula through a tiny incision to suction out fat and tighten tissue. Patients can achieve noticeably younger-looking arms from liposuction with short recovery times and often only a local anesthetic required. It is important for patients to maintain realistic expectations and use liposuction for minor fat reduction and shaping rather than for significant weight loss. Moreover, arm liposuction gets best results for patients whose skin is able to bounce back. An arm lift (brachioplasty) may be a better option if your skin has lost elasticity.

• Lower Body Lift

  A body lift, also known as a belt lipectomy, is performed to remove excess skin or fatty tissue on the body that results from age or extreme weight loss. More extensive than just a tummy tuck, the body lift is performed on the lower torso and upper legs and resculpts the abdomen, hips, back, buttocks and outer thighs. Why Get A Body Lift? Exercise and a healthy diet cannot always achieve the desired results for people who have loose, sagging skin and uneven contours. Aging, sun damage, cellulite, genetics, pregnancy, and significant fluctuations in weight may contribute to poor tissue elasticity and can result in sagging of the abdomen, buttocks, and thighs. A body lift may help to achieve a firmer, more youthful-looking body. Candidates For Body Lift People who have lost a large amount of weight, either through gastric bypass surgery, intestinal bypass surgery, lap-band surgery or change in nutrition and fitness habits, may have excess skin on their bodies and are excellent candidates for a body lift. Other people who are unable to lose 30 or 40 pounds through diet or exercise may also opt for body lift surgeries. A body lift is also an option for normal-weight women who have given birth and want to remove remaining skin and tissue after losing post-pregnancy weight. The Body Lift Procedure The steps of the body lift procedure differ depending on the needs of the patient. In most cases, however, the surgeon will operate on the stomach first, removing excess skin from the belly button to the pubic area and then tightening the abdominal wall muscles. This is the area that is most often loosened by pregnancy or excessive weight loss. The fat and skin from above the belly button is pulled down and sutured in place, and the belly button is pulled out to its normal position. The procedure continues with the buttocks and thigh areas, with the incisions being made as inconspicuous as possible. In cases that require the removal of fat, liposuction is often involved. The entire body lift procedure takes 3 to 6 hours and is performed under general anesthetic. Recovery A body lift is a major surgical procedure and requires significant recovery time. Most patients can expect to remain in the facility for 1 to 3 nights after the operation and can typically return to work in 2 to 4 weeks. Patients are encouraged to walk after the surgery, but to avoid other activities such as exercising until sufficient healing has occurred. This usually takes 6 to 8 weeks. Approximately 75 percent of the swelling goes down after six weeks and 90 percent after about three months. Risks And Considerations As with every surgical procedure, a body lift has risks and potential complications. These include: Fluid accumulating under the skin (seroma) Small skin separations Reduced sensation in the treated area Infection Bleeding (hematoma) Blood clots Unfavorable scarring Skin discoloration Recurrent looseness of skin Deep vein thrombosis, cardiac and pulmonary complications

• Buttock Lift

  As we get older, our buttocks and thighs may begin to droop and sag like other areas of the body, such as the breasts and stomach. Stubborn pockets of localized fat may develop as well, which do not go away despite traditional weight loss attempts such as diet and exercise. Fortunately, like these other areas, there are surgical options available for reversing these changes and restoring a tighter, firmer and more youthful appearance to the buttock and outer thigh area.These cosmetic procedures can be performed alone, or in combination with other cosmetic procedures, such as part of a body lift or with a gluteal augmentation. When combined with other procedures, patients can achieve maximum aesthetic results and enjoy a younger and more refreshed appearance overall. Candidates For The Buttock Lift Procedure The buttock lift procedure is ideal for candidates who are in good general health and have excess skin and tissue within the area, as a result of the natural aging process, pregnancy or significant weight loss. This procedure should not be considered an easy weight loss solution, and should be performed on patients who are close to their ideal weight, but are unhappy with the appearance of their buttocks and outer thighs. The buttock procedure is performed on an outpatient basis under general anesthesia. The entire procedure usually takes two to three hours to perform. Your doctor will place the incision across the top of the buttocks, low enough to be concealed by underwear or a bikini. This incision spreads along the back from one hip bone to the other. After the incision is made, excess skin and fat are removed from the buttocks and thighs. This may be done using liposuction if there is a large amount of fat to be removed. After the buttock lift procedure, patients may experience temporary swelling, soreness and bruising for a few days. These side effects can be controlled with pain medication prescribed by your doctor. Most patients can return to work and regular activities after about two weeks, but should avoid strenuous exercise for at least four weeks. The appearance of the scars from this procedure will continue to change for up to 18 months, but usually fade and can be easily concealed.   Liquid rhinoplasty is a procedure in which injectable fillers are used to alter and sculpt a patient’s nose without invasive surgery. The most commonly used fillers are hyaluronic acid (Restylane, Juvederm, Voluma) or calcium hydroxyapatite (Radiesse). The injected liquid “fills” the depressed areas on the nose, lifting the tip or smoothing the bumps on the nasal bridge. Liquid rhinoplasty or non-surgical rhinoplasty is a cosmetic procedure and rarely corrects functional or breathing problems of the nose. Results usually last six months to one year depending on which fillers are used, and the procedure may be repeated.

  Lip augmentation is a cosmetic procedure that, either surgically or with injections, increases lip size. Either the upper or lower lip, or both, can be augmented. Whether the results are temporary or long-lasting depends on the type of augmentation performed. There are several different techniques for augmenting lips. Most involve injecting a facial filler, although fat transfers (also called fat grafting) and implants are also options. Injectable fillers usually contain a hyaluronic-acid material. Fat transfers inject fat that is harvested from the patient’s body. Implants are placed under the skin; they are often made of a soft, solid-silicone elastomer. Which type of augmentation is used depends upon the individual patient, doctor preference and the cosmetic results desired.

Unlike the original JUVÉDERM injectable gel, JUVÉDERM XC is infused with lidocaine to make its injection more comfortable. JUVÉDERM XC is a smooth-consistency gel that restores volume to the skin, and smooths away moderate-to-severe facial wrinkles and folds. It is made of hyaluronic acid (HA), a sugar that occurs naturally in the body. In terms of the skin, HA’s role is to deliver nutrients and hydration, and act as a cushioning agent. Young, healthy skin contains a lot of HA, but factors that include aging and sun exposure reduce the amount, causing skin to lose structure and volume. JUVÉDERM XC is most effective in improving the appearance of nasolabial folds, the lines that run from the bottom of the nose to the corner of the mouth on both sides of the face. RADIESSE filler is composed of microspheres in a water-based gel; the microspheres are made of a substance called calcium hydroxylapatite, which stimulates the production of collagen. • Laser Skin Resurfacing

  Modern technology provides a variety of successful laser resurfacing treatments for improving the condition of the skin. Laser skin treatments employ focused beams of light energy to counteract the effects on the skin of acne, aging, sun damage, hyperpigmentation, scars, or poor nutrition. Resurfacing laser treatments are safe and efficient, requiring little downtime to provide effective results. Much less invasive than dermabrasion and chemical peels, laser treatments remove damaged skin and rejuvenate the patient’s appearance. Most of the time, such treatments are sought for the face, but may also be performed on other parts of the body. Reasons For Laser Resurfacing Patients seek laser resurfacing for a variety of reasons, all related to discontent with their appearance. Laser resurfacing may be performed for any of the following reasons: To decrease or eliminate wrinkles or stretchmarks To diminish the appearance of scars To rejuvenate non-responsive skin after a facelift To remove age spots or spider veins To repair sun-damaged skin To improve skin tone due to melasma or other causes To remove warts or birthmarks To decrease the size of enlarged pores To remove tattoos Types Of Laser Resurfacing There are two basic types of laser resurfacing treatments: ablative and non-ablative. Ablative Laser Resurfacing Ablative laser treatments, which penetrate more deeply, actually cause a wound, since they remove a layer of skin. Since ablative resurfacing actually vaporizes a layer of the skin, it provides more dramatic results and requires fewer sessions, but also requires a longer period of healing. Non-Ablative Resurfacing Non-ablative treatments do not burn away skin tissue, but use laser energy to stimulate collagen growth and tighten underlying skin. Non-ablative resurfacing is gentler than ablative and requires less downtime, but may necessitate more treatment sessions and may yield less dramatic results. In general, non-ablative laser treatments are used when the repair needed is less extensive. Also, non-ablative treatments are recommended for people of color for whom ablative resurfacing may be more likely to cause unsightly changes in pigmentation. A form of ablative resurfacing known as fractional ablative laser is now increasingly in use. This procedure targets more specific regions so that laser microbeams pinpoint and peel away smaller areas of skin. During fractional laser surfacing, no visible wounds are caused, so, although results are more noticeable, less downtime is necessary than after conventional ablative treatments. For some individuals, fractional ablative laser resurfacing represents the best of both worlds. The Laser Resurfacing Procedure Laser treatments for skin conditions are typically performed in the doctor’s office with topical anesthesia. Procedures typically take between 30 and 90 minutes to perform, depending on the size and location of the treated area. While some individuals will notice significant results after just one treatment, particularly with ablative procedures, many patients will require multiple sessions in order to achieve the desired results. It is also possible that additional touch-up treatments will be needed at a future date. Risks Of Laser Resurfacing Although laser skin treatment is considered safe for most patients and is typically performed with no long-term complications, there are certain risks associated with any type of medical procedure. While considered rare, risks may include: Infection Scarring Redness Pigmentation changes Acne Milia, tiny white bumps on the skin Recovery From Laser Resurfacing Recovery from laser resurfacing procedures varies according to the particular procedure undergone and the individual patient’s skin type. In all cases, however, patients who have undergone these treatments should avoid sun exposure wherever possible. Recovery From Non-Ablative Laser Resurfacing After treatment, patients may experience redness and peeling at the treated site as the skin heals. The duration of these side effects varies, but is commonly several days. Ice packs may ease discomfort and improve appearance. Normal activities can be resumed during healing, and makeup may be applied to camouflage any unwanted, evidence of treatment. Recovery From Ablative Laser Resurfacing After ablative laser resurfacing, the treated skin will temporarily be raw, swollen and itchy and the yellowish liquid that oozes from treated areas may form crusts. After treatment, a thick ointment, such as petroleum jelly, and a watertight dressing will be applied to the affected area. If treatment has been performed on the face, the patient may be instructed to keep the head elevated during the night. Ice packs and over-the-counter pain relievers may be recommended for symptom relief. To help avoid scarring, patients are instructed not to pick at crusts or pieces of skin. Visible healing may take a week or two.
